What a notice needs

• Your name, address and email.
• Identification of the work you say is infringed.
• The exact URL on this site that carries it.
• A statement that you believe in good faith that the use is not authorised.
• A statement, under penalty of perjury, that the notice is accurate and that you may act for the owner.
• Your physical or electronic signature.

Counter-notice

If your material was removed and you believe that was a mistake, send a counter-notice to the same address with the same contact details, the material removed, and a statement under penalty of perjury that the removal was an error.

Repeat infringers lose access. If you only want your domain excluded rather than a formal notice, the opt-out page is faster.
